PARTICIPANT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK

PLEASE READ CAREFULLY BEFORE SIGNING. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.

1. Assumption of Risk. I understand that GellyBall / gel blaster activities ("the Activities") involve inherent risks, including being struck by gel projectiles, eye or skin injury, collisions with equipment, inflatable bunkers or other participants, slips, trips and falls, and other bodily injury, illness or property damage. I voluntarily choose to participate with full knowledge of these risks.

2. Release of Liability & Indemnification. In consideration of being permitted to participate, I, on behalf of myself, my heirs, personal representatives and assigns, release, waive, discharge and covenant not to sue Big Time Fun Company, its owners, officers, employees, agents, volunteers, and the owners/lessors of the premises, from any and all liability arising out of any loss, damage or injury, including death, whether caused by the negligence of the released parties or otherwise, except for gross negligence or willful misconduct.

3. Rules Acknowledgment. I agree to wear all required protective equipment (including eye protection) at all times in the play area, to follow all posted rules and staff instructions, and to exit the play area when hit or when "game over" is called.

4. Medical Treatment Authorization. I authorize Big Time Fun Company staff to obtain emergency medical treatment on my behalf, or on behalf of any minor listed above, and agree to be financially responsible for the cost.

5. Minor Participants. If a participant is under 18, this release must be read and signed by a parent or legal guardian, who agrees to all terms on the minor's behalf and confirms they have the legal authority to do so.

This release is valid for one (1) year from the date signed.

Photos and video of me may be used to promote Big Time Fun Company. I am the parent or legal guardian of each child listed above, and I have the legal authority to sign on their behalf. I have read and agree to the release above, for myself and for each child listed.
